This essay analyzes two types of patient‐experience data to broaden and deepen understanding of trust in health care. Analysis of patients’ open‐ended comments shows a close connection between patients’ feelings of trust and their intent to recommend providers and provider organizations—a global measure to evaluate patients’ perceptions of care experiences. Patients’ comments also reveal the bidirectional building of trust between the patient and the caregiver. Trust gets built when patients perceive their caregivers to trust their knowledge of their bodies as (...) well as when caregivers demonstrate caring behaviors that earn the patients’ trust. Patients’ ratings of a closed‐ended survey item on “confidence in provider” create the greatest differentiation for the global measure of patient experience—whether patients did or did not recommend a practice or provider. The essay also discusses related findings on pre‐visit friction and the use of humor by the caregiver to expand understanding of trust. (shrink)

Our book Relevance (Sperber and Wilson 1986) treats utterance interpretation as a two-phase process: a modular decoding phase is seen as providing input to a central inferential phase in which a linguistically encoded logical form is contextually enriched and used to construct a hypothesis about the speaker's informative intention. Relevance was mainly concerned with the inferential phase of comprehension: we had to answer Fodor's challenge that while decoding processes are quite well understood, inferential processes are not only not understood, but (...) perhaps not even understandable (see Fodor 1983). Here we will look more closely at the decoding phase and consider what types of information may be linguistically encoded, and how the borderline between decoding and inference can be drawn. It might be that all linguistically encoded information is cut to a single pattern: all truth conditions, say, or all instructions for use. However, there is a robust intuition that two basic types of meaning can be found. This intuition surfaces in a variety of distinctions: between describing and indicating, stating and showing, saying and conventionally implicating, or between truth-conditional and non-truth-conditional, conceptual and procedural, or representational and computational meaning. In the literature, justifications for these distinctions have been developed in both strictly linguistic and more broadly cognitive terms. The linguistic justification goes as follows (see for example Recanati 1987). Utterances express propositions; propositions have truth conditions; but the meaning of an utterance is not exhausted by its truth conditions, i.e. the truth conditions of the proposition expressed. This article looks at the feminist activism of particular women in the ancestry of the eminent Canadian sociologist, Dorothy E. Smith, and at the archival data that confirm the traces of their influence found in her theory-building. Using the method of interpretative historical sociology and a conceptual framework drawn from Marx called the `productive forces', the article examines the feminist theology of her Quaker ancestor, Margaret Fell, and the militant suffrage activism of her mother and her grandmother, Dorothy Foster Place (...) and Lucy Ellison Abraham, respectively. The article argues that the household labour of the remarkable women in her family line became a `productive force' that facilitated her imagining of the feminist theory, `the standpoint of women'. Background Since the UK Abortion Act (1967), women have travelled from Ireland to the UK for legal abortion. In 2011 >4000 women did so. Knowledge and attitudes of medical students towards abortion have been published, however, this is the first such report from Ireland. Objective To investigate medical students’ attitudes towards abortion in Ireland. Methods All medical students at the University of Limerick, and physicians who graduated from the university within the previous 12 months, were invited via email to complete (...) an anonymous online survey. The questionnaire comprised 17 questions. Quantitative and qualitative analyses were performed. Results Response rate was 45% (n=169; 55% women; 88.2% <30 years of age; 66.7% Irish; 29.2% North American). Outcomes were: abortion should not be legally available (7.1%), abortion should be allowed in limited circumstances only (35.5%), abortion should be legally available upon request (55%). 72.8% of respondents were moderately/strongly prochoice (74% of women/71% of men/72% and 76% of Irish and North American respondents, respectively). Students aged >30 years were less likely to be prochoice (55%). While 95.2% believed that education on abortion should be offered within medical school curricula, 28.8% stated that they would decline to terminate pregnancies even if legally permitted. While 58.8% indicated that they might perform legal abortions once qualified, 25.7% would do so under limited circumstances only. Conclusions The majority of participants wanted education regarding abortion. Despite being predominantly prochoice, considerably fewer students, irrespective of nationality, indicated that they would perform abortions. (shrink)

Shaping Public Theology, which is also edited by Paeth along with E. Harold Breitenberg Jr. and Hak Joon Lee, is a helpfully representative compilation of Stack-house’s own writing across the span of his long and prolific career. As such, it demonstrates both the generous aspirations and the significant limitations of “public theology” as Stackhouse envisioned it, as a theologically informed discourse that should be equally accessible to Christians, people of other faiths, and people of no faith or religious background. [End Page 213]The first of the book’s three sections, “Interpreting the Tradition of Public Theology,” places Stackhouse in conversation with other figures in Christian theology and ethics, both past and present. These essays locate him firmly within the strand of Christian thought inhabited by Ernst Troeltsch, Walter Rauschenbusch, and Paul Tillich, and they also reflect his significant and ongoing disagreements with some of his contemporaries, including Alasdair MacIntyre and Stanley Hauerwas. The second section, “Developing a Method for Public Theology,” provides a useful summary of Stackhouse’s distinctive theological method, defining public theology and its relationship to individuals, communities, institutions, and states. The third and final section, “Constructing a Global Public Theology,” turns to one of the major focal points of Stackhouse’s work, examining the role of public theology in the context of globalization. The text concludes with a brief summary contributed by the editors, a response from Stackhouse describing both how his thought has changed over the years and the gaps that he perceives in his work, and a comprehensive bibliography.As a collection, Shaping Public Theology provides the reader with a broad overview of the predominant themes in Stackhouse’s work. The essays demonstrate his abiding concern for an apologetic and universal framework for Christian ethics as well as his emphasis on the covenantal basis for that framework. Stackhouse’s affinity for both the Reformed tradition and the ideals of the Social Gospel movement are also evident throughout the text. In addition many of the essays reflect his ongoing engagement with the theme of “the global” and with globalization not only as a locus of critique but also as the vehicle for “the prospect of a world-comprehending culture, with a new recognition of certain common moral standards” (162).Whether these ideas are theologically helpful, however, is a matter of long-standing disagreement that will not be resolved in the limited space of this review. By treating the covenant between the Triune God and Israel as merely one instance of a “universal moral law,” and by moving from that particular to the universal through the doctrine of the Trinity, Stackhouse participates in a significant—and significantly problematic—strain of thought within the Christian imaginary (67). While he may reject the particularity espoused by George Lindbeck, Hauerwas, et al., he remains grounded within his own particularity with its own decidedly fraught history: that of the modern, progressive, Western civilizing project. As a work of moral theology, this text does have significant limitations. Deirdre Wilson provides a reflective overview of a volume devoted to the historic application of relevance-theoretic ideas to literary studies. She maintains a view argued elsewhere that the putative non-propositional nature of literary effects are an illusion, a view which dates to Sperber and Wilson : “If you look at [non-propositional] affective effects through the microscope of relevance theory, you see a wide array of minute cognitive [i.e., propositional] effects.” This paper suggests an alternative, that modern-day humans have two (...) apparently different modes of expressing and interpreting information: one of these is a system in which propositional, cognitive effects dominate; the other involves direct, non-propositional effects. The paper concludes by describing two ways such affects might be assimilated into relevance theory. The first, to accept that humans are much more than merely cognitive organisms; the second, to rethink quite radically what we mean by cognition. (shrink)

Humans massively depend on communication with others, but this leaves them open to the risk of being accidentally or intentionally misinformed. To ensure that, despite this risk, communication remains advantageous, humans have, we claim, a suite of cognitive mechanisms for epistemic vigilance. Here we outline this claim and consider some of the ways in which epistemic vigilance works in mental and social life by surveying issues, research and theories in different domains of philosophy, linguistics, cognitive psychology and the social sciences.

Growing concern has been expressed that we have entered a “post-truth” era in which each of us willfully believes whatever we choose, aided and abetted by alternative and social media that spin alternative realities for boutique consumption. A prime example of the belief in alternative realities is said to be acceptance of “conspiracy theories”—a term that is often used as a pejorative to indict claims of conspiracy that are so obviously absurd that only the unhinged could believe them. The epistemological (...) standard often involved in this indictment, however—the standard of “obvious” falsity—invites subjectivity in its application, because what is obviously false to one person can be common sense to another. This is not just a truism; considerable research suggests that people’s political beliefs, in general, and their acceptance or rejection of conspiracy theories in particular, tends in large part to be determined by partisan, ideological, and other priors. When people speak, their words never fully encode what they mean, and the context is always compatible with a variety of interpretations. How can comprehension ever be achieved? Wilson and Sperber argue that comprehension is an inference process guided by precise expectations of relevance. What are the relations between the linguistically encoded meanings studied in semantics and the thoughts that humans are capable of entertaining and conveying? How should we analyse literal meaning, approximations, metaphors and ironies? Is the ability to (...) understand speakers' meanings rooted in a more general human ability to understand other minds? How do these abilities interact in evolution and in cognitive development? The title of this paper reflects the fact truthmaking is quite frequently considered to be expressive of realism. What this means, exactly, will become clearer in the course of our discussion, but since we are interested in Armstrong’s work on truthmaking in particular, it is natural to start from a brief discussion of how truthmaking and realism appear to be associated in his work. In this paper, special attention is given to the supposed link between truthmaking and realism, but it (...) is argued that this link should not be taken too seriously, as truthmaking turns out to be, to a large extent, ontologically neutral. It is now possible to have a child without sexual intercourse. The history of this apparently new statement has been breefly synthetised.The new perspectives open ethical problems for the scientific community and a new social context for single women.

The main aim of this paper is to introduce the logics of evidence and truth $$LET_{K}^+$$ and $$LET_{F}^+$$ together with sound, complete, and decidable six-valued deterministic semantics for them. These logics extend the logics $$LET_{K}$$ and $$LET_{F}^-$$ with rules of propagation of classicality, which are inferences that express how the classicality operator $${\circ }$$ is transmitted from less complex to more complex sentences, and vice-versa. The six-valued semantics here proposed extends the 4 values of Belnap-Dunn logic with 2 more values (...) that intend to represent (positive and negative) reliable information. A six-valued non-deterministic semantics for $$LET_{K}$$ is obtained by means of Nmatrices based on swap structures, and the six-valued semantics for $$LET_{K}^+$$ is then obtained by imposing restrictions on the semantics of $$LET_{K}$$. These restrictions correspond exactly to the rules of propagation of classicality that extend $$LET_{K}$$. The logic $$LET_{F}^+$$ is obtained as the implication-free fragment of $$LET_{K}^+$$. We also show that the 6 values of $$LET_{K}^+$$ and $$LET_{F}^+$$ define a lattice structure that extends the lattice L4 defined by the Belnap-Dunn four-valued logic with the 2 additional values mentioned above, intuitively interpreted as positive and negative reliable information. Finally, we also show that $$LET_{K}^+$$ is Blok-Pigozzi algebraizable and that its implication-free fragment $$LET_{F}^+$$ coincides with the degree-preserving logic of the involutive Stone algebras. Argues that there can be interaction without breaking physical laws: e.g. by basic psychic forces, or by varying physical constants, or especially by arranging fractal trees of physical causation leading to behavior.

The interpretation of metaphorical utterances often results in the attribution of emergent properties, which are neither standardly associated with the individual constituents in isolation nor derivable by standard rules of semantic composition. An adequate pragmatic account of metaphor interpretation must explain how these properties are derived. Using the framework of relevance theory, we propose a wholly inferential account, and argue that the derivation of emergent properties involves no special interpretive mechanisms not required for the interpretation of ordinary, literal utterances.

Between 1873 and 1876, Felix Klein published a series of papers that he later placed under the rubric anschauliche Geometrie in the second volume of his collected works (1922). The present study attempts not only to follow the course of this work, but also to place it in a larger historical context. Methodologically, Klein’s approach had roots in Poncelet’s principle of continuity, though the more immediate influences on him came from his teachers, Plücker and Clebsch. In the 1860s, Clebsch reworked (...) some of the central ideas in Riemann’s theory of Abelian functions to obtain complicated results for systems of algebraic curves, most published earlier by Hesse and Steiner. These findings played a major role in enumerative geometry, whereas Plücker’s work had a strongly qualitative character that imbued Klein’s early studies. A leitmotif in these works can be seen in the interplay between real curves and surfaces as reflected by their transformational properties. During the early 1870s, Klein and Zeuthen began to explore the possibility of deriving all possible forms for real cubic surfaces as well as quartic curves. They did so using continuity methods reminiscent of Poncelet’s earlier approach. Both authors also relied on visual arguments, which Klein would later advance under the banner of intuitive geometry (anschauliche Geometrie). (shrink)

The thought that empathy plays an important role in moral motivation is almost a platitude of contemporary folk psychology. Parallel themes were mooted in German moral philosophy and aesthetics in the 1700s, and versions of the empathy construct remained prominent in continental accounts of moral motivation through the nineteenth century and early twentieth centuries. This chapter elucidates the Empathic Motivation Hypothesis (EMH) and sets out some of the conceptual and empirical challenges it faces. It distinguishes empathic concern from other dimensions (...) of empathy (e.g., resonance, attunement, distress, and non-empathic concern) and examines the merits of EMH as a developmental thesis, focusing on evidence from psychopathology and attachment theory. The main scientific problems of chemical bonding were solved half a century ago, but adequate philosophical understanding of chemical combination is yet to be achieved. Chemists routinely use important terms ("element," "atom," "molecule," "substance") with more than one meaning. This can lead to misunderstandings. Eliminativists claim that what seems to be a baseball breaking a window is merely the action of "atoms, acting in concert." They argue that statues, baseballs, and similar macroscopic things "do not exist." When macroscopic objects like (...) baseballs move, exceedingly large numbers (1025) of microscopic components coordinate their activities. Understanding how this happens requires attention to the interactions that link parts into larger units. Eliminativists say that everything that truly exists has causal relationships in addition to those of its components—"nonredundant causality." This paper holds that if a number of entities interact in such a way that the effect of that collection on test objects is different than it would have been in the absence of the interaction, then identification of that collection as a single composite agent is warranted, for purposes to which that difference is relevant. Ordinary "chemical substances" (both elementary materials such as dihydrogen and compounds such as water) fulfill this version of the requirement of nonredundant causality. Other sorts of chemical coherences, including chemical dissipative structures (e.g., flames), also fulfill that criterion. All these types of coherences qualify as "substances" (as that term is used in philosophy) even though they are not all "chemical substances.". (shrink)
